Should any Club need to make a claim, please follow the procedure of downloading and completing the claim form as instructed and post the completed form to the following Northern Football Alliance Official:

John oversees the claims procedure and we found that with having a dedicated league official controlling the system, it prevents any confusion and minimises any problematic issues.

It is important that any sickness form relating to an injury either at the beginning of a claim, during a claim and at the end of the claim MUST always go to John,
who will then log the information and personally send it on to the Claims Department of our Insurance Company.
